Your Privacy is Our Concern

Certain laws regulate the collection, use, and disclosure of a consumer or customer's nonpublic information. The Spectera Family of Companies does not sell or otherwise disclose any nonpublic personal information about our customers or former customers to anyone outside the Spectera family, except as permitted by law. You do not have to take any action, this notice is solely for your information.

It is the policy of the Spectera Family of Companies to:

  • Collect only information necessary or relevant to our business.
  • Make a reasonable effort to ensure that information we act upon is accurate, relevant, timely and complete.
  • Use only legitimate means to collect information.
  • Make personal information available externally only to respond to legitimate business needs, to regulatory or other government authorities or as otherwise permitted by law.
  • Limit employees' access to those who need access to that personal information.

We are providing you with the following summary of the kinds of information that the Spectera Family or its agents may collect, what is done with information after it is collected, and how you can find out about information, if any, we have about you in our records.

What kind of information do we collect about you and from whom?

Vision Plans
If you are a vision policyholder or an enrollee in a Spectera Family vision plan, we get most of our information directly from you. The enrollment information generally gives us most of the information we need to know. Additionally, the provider will inform us when you receive services and which services were received. This may include lens prescription information. Sometimes we may contact you to obtain additional information.

Dental Plans
Spectera Family's dental plans are administered by a third party which has access to information about you, your eligibility, and services received. This third party makes personal information available externally only to respond to legitimate business needs, to regulatory or other government authorities or as otherwise permitted by law and limits employees' access to those who need access to that personal information. Entities in the Spectera Family have access to this information to verify that the correct coverage is being provided and that the billing is accurate.

Medical Necessity
Spectera, Inc. is the only member of the Spectera Family that is involved in making medical necessity determinations. The information required to make medical necessity determinations may be gathered from you, your medical records, and in very rare cases, from your family. Depending on the nature of your medical condition, we may need additional information from outside sources, such as court or other public records and such as other insurance companies which may have provided previous coverage.

What do we do with the information collected about you?

Any information we have collected about you will be stored in one of the following: our vision plan records, our dental plan records, our utilization review records, or in your agent's files. We refer to and use information in our dental and vision plan records for purposes related to establishing and administering the benefits and settling claims. The information in the utilization review files is used to make medical necessity determinations and administer the utilization review program. Your agent may use information about you in his/her files for insurance marketing purposes or to help you with your overall insurance program.

To whom do we disclose information about you?

We will not disclose information about you to others without your written consent unless the disclosure is part of the normal conduct of our business. By law, we are permitted to share information about you without prior permission under certain circumstances to certain persons and organizations such as:

  • Our affiliated insurance companies
  • Your agent or broker
  • Parties who perform a business, professional or insurance function for our company, including our third party administrators
  • Businesses that conduct actuarial or underwriting studies
  • Other insurance companies, or agents as reasonably necessary in connection with any application, policy, or claim involving you
  • Insurance support organizations which are established to collect information for the purpose of detecting or preventing insurance crimes or fraudulent claims
  • Insurance regulatory or law-enforcement agencies in connection with the regulation of our business

Vision policy information is normally archived for 5 years. Dental policy information is normally archived for 5 years. Utilization management (medical necessity determination) information is normally archived for 7 years. AT ANY TIME, YOU HAVE THE RIGHT TO REMOVE YOURSELF FROM OUR DATABASES BY CONTACTING US IN WRITING.

How do we protect the confidentiality of information about you?

We restrict access to nonpublic information about you to those employees who need to know that information to provide products or services to you. We maintain physical, electronic, and procedural safeguards that comply with federal regulations to guard your nonpublic personal information.

How can you find out about information we have about you?

You have the right to know what kind of information we keep in our files about you, to have reasonable access to it and receive a copy. Write to us if you have questions about information that you would like to receive. When you write to us, please provide your complete name and address, state whether you received vision coverage, dental coverage, or received a medical necessity determination, provide your social security number and insured group (usually your employer or union) and identify the information you seek.
