Regulatory Information by State for Group Prepaid Vision Only Plans
Group prepaid vision plans are underwritten in different states by different
companies which were formed and licensed on different dates. Because each company
is a separate legal entity, unless otherwise stated, funding to provide the prepaid
vision services comes only from the company that provides or underwrites the prepaid
vision services in that state. Unless otherwise specified, all statements on this
web site are intended to refer to Spectera, Inc.
Spectera, Inc. provides prepaid vision coverage in Alabama, Alaska, Delaware,
District of Columbia, Georgia, Hawaii, Kansas, Louisiana, Missouri, Montana, New Jersey,
Rhode Island, South Carolina, and Wyoming.
Spectera, Inc. administers prepaid vision care underwritten
by American General Life Insurance Company in the State of Pennsylvania
(referred to as American General of Pennsylvania) in Arizona,
Arkansas, Colorado, Connecticut, Idaho, Illinois, Iowa, Kentucky,
Massachusetts, Michigan, Minnesota, Mississippi, Nebraska, Nevada,
New Hampshire, New Mexico, New York, North Dakota, Ohio, Oklahoma,
Oregon, Pennsylvania, Tennessee, Utah, Vermont, Washington, West
Virginia, and Wisconsin.
In California, prepaid vision coverage may be obtained from either Spectera Vision Services of California,
Inc. or American General of Pennsylvania.
Spectera Vision Services of Florida, Inc., A Prepaid Limited Health Services Organization
licensed under Chapter 636 provides prepaid vision services in Florida.
Spectera Insurance Company provides prepaid vision services in Maryland.
Spectera Insurance Company, Inc. provides prepaid vision services in Texas.
Spectera Vision, Inc. provides prepaid vision services in Virginia and Indiana through the following
policies: gk-cmb-svi.400, gk-stor-cmb-svi.400, gk-svi.400, gk-stor-svi.400, gk-stor-svi.200,
gk-stor-svi.200, gk-stor-cmb-svi.1299, gk-stor-svi.1299, gk-cmb-svi.1299, and gk-svi.1299.
Prepaid vision services in all states are provided only on a group basis. Vision coverage
is provided only for a vision examination and a pair of eyeglasses or (at the client's option)
a set of contact lenses in lieu of eyeglasses every 12 or 24 months. Co-payments may be
required, depending on the options chosen. Some eyeglasses, eyeglass lenses, lens options,
or contacts may not be covered or may be partially covered, depending on the options chosen.
No medical benefits are provided. Benefits vary with the options chosen and costs vary with
the benefit options chosen.
